Cardiac arrest is the one canine emergency where there is no time to look anything up. When a dog's heart stops, you have a window measured in minutes, and what happens in that window usually decides the outcome. Every dog owner should know the basic mechanics of CPR — but just as much, you should know its limits. CPR almost never fixes the underlying problem by itself. What it does is keep blood moving to the brain and heart while you get to the nearest clinic, and that only works if someone is driving while you compress. Think of it as buying time, nothing more and nothing less.

Perform CPR on a dog only when the dog is unconscious, not breathing, and has no detectable heartbeat or pulse — if any breathing or pulse is present, skip compressions and go straight to a vet. Give chest compressions at 100–120 per minute, pressing down one-third to one-half the width of the chest and letting it fully recoil between pushes. After every 30 compressions, give 2 rescue breaths into the nostrils, working in cycles of about two minutes before a quick reassessment. CPR buys time but does not replace emergency veterinary care — have someone call the nearest clinic while you start, and transport as soon as you can.

When should you perform CPR on a dog?

CPR is for one situation only: cardiac arrest. All three of the following have to be true before you start compressions:

  • Unconscious. The dog does not respond to being called, shaken gently, or touched. A dog that is merely weak or collapsed but still aware of you is not a CPR candidate.
  • Not breathing. Watch the chest for movement and hold your hand near the nostrils for airflow. Occasional gasping (agonal breathing) does not count as real breathing — it is a sign of arrest, not a reason to hold off.
  • No heartbeat or pulse. Feel the femoral artery on the inside of the thigh, or press your palm against the chest just behind the left elbow. If the dog still has a pulse or is breathing, do not do chest compressions. Compressing a beating heart can cause real injury and buys nothing. In that situation, the right move is rapid transport to a vet while you keep the airway clear.

Dog CPR step by step

Veterinary CPR has an established consensus framework, and the pet-owner version of it comes down to five numbers worth memorizing:

Position the dog. Lay the dog on its right side on a firm surface — a floor, not a bed or couch. For most dogs, you compress the chest at its widest point, roughly where the elbow meets the body when the leg is bent back. Barrel-chested breeds like bulldogs can also be positioned on their back with you pressing the breastbone, which takes advantage of the chest acting as a pump. For wide-chested large dogs, press directly over the heart.

Compress hard and fast. Lock your elbows, stack your shoulders over your hands, and push at 100–120 per minute — the rhythm of a brisk disco beat, if that helps. Each push should sink the chest by a third to a half of its width, and each release should let the chest spring back fully. Leaning on the chest between compressions is the most common mistake; it keeps the heart from refilling. For small dogs and puppies, use one hand or cup the chest with your fingers and thumb rather than your full body weight — same rate, gentler depth.

Work in two-minute cycles. After every 30 compressions, pause just long enough to give 2 breaths, then get straight back on the chest. Every two minutes or so, take a few seconds to recheck for a pulse and breathing. Frequent long pauses undo your work, so keep checks brief.

How to give rescue breaths

Hold the dog's mouth closed with one hand, seal your lips over the nostrils, and blow steadily until you see the chest rise. If the chest does not move, check the airway for a visible obstruction and re-seal over the nose. Two breaths, then back to compressions — breathing for the dog matters, but compressions matter more, and extended breaks to fiddle with the airway cost more than they gain.

In a clinic, the same job is done with a bag-valve-mask or an endotracheal tube connected to oxygen, which delivers far more oxygen than the depleted air in your exhaled breath. That gap between home rescue breathing and clinical oxygen therapy for dogs is one more reason CPR is a bridge, not a destination.

Getting to the vet: why CPR alone isn't enough

Here is the honest part: survival is unfortunately low, even in hospital settings with a full team, drugs, and monitoring. Out-of-hospital arrest, where an owner is improvising in a living room, is harder still. That is not a reason to skip CPR — dogs do come back, and compressions are often what keeps the brain alive long enough for a vet to have something to work with. It is a reason to treat CPR as step one of a two-step plan, where step two is a moving car.

If someone else is home, split the jobs immediately: one person starts compressions, the other calls the nearest emergency clinic so a team is ready when you arrive, then drives. Compressions can continue in the back seat or cargo area of a car. If you are alone, do a cycle or two, then move the dog to the car and alternate driving and pulling over — imperfect, but better than staying home.

What your vet will do

The clinic version of CPR runs on equipment you cannot replicate at home. An ECG reading tells the team whether the heart's rhythm is one that responds to electrical shock or one that needs medication — a distinction that changes everything about the next few minutes. End-tidal CO₂ monitoring, measured with a capnography-capable patient monitor, gives an objective readout of whether chest compressions are actually generating blood flow, so the team can correct technique in real time instead of guessing. An endotracheal tube goes in, connected to oxygen delivery equipment, and an intravenous catheter opens a line for emergency drugs like epinephrine and for fluids. Blood work and imaging follow if the dog stabilizes, because something caused the arrest — a heart rhythm problem, internal bleeding, heatstroke — and CPR treats none of those.

How to prevent an emergency in the first place

The arrest you survive is usually the one that never happens, because the dog reached a vet while there was still a pulse to treat. Learn the warning signs that precede collapse and act on them early: episodes of sudden weakness or collapse in dogs, labored breathing, gums that turn pale or bluish, and overheating — heatstroke in dogs is one of the more preventable paths to a crash. A dog showing any of these needs a clinic now, not tomorrow. CPR skill matters, but the habit that saves more dogs is leaving early.
