One-handed operation

Long press turns it on and off; short press adjusts light source brightness. Short press captures an image, long press starts video. Remaining power shows on screen.

Every one of those is a one-handed action, which matters because your other hand is holding an animal that would rather be somewhere else. A device requiring two hands to operate needs a second person, and a second person isn't always available in a busy consulting room.

Detachable lens and what it becomes

TT-OTS100 or TT-OTS200?

The TT-OTS200 is the larger sibling — a digital video otoscope, rhinoscope and laryngoscope with a 4.5-inch touch screen and a full set of accessories.

Its accessory range is where the real difference sits. Field of view determines how much canal wall you see at once, and 78° is a working compromise rather than a wide-angle view.

In a straight canal that's plenty. That's not a criticism of this device specifically; it's the physical reality of handheld video otoscopy.
